Documentation should verify any extenuating circumstances that occurred after the deadline and prevented you from changing your schedule in a timely manner. Complete and legible documentation must be submitted with your appeal. Submission of documentation is required for consideration but does not guarantee approval of your request.
Medical documentation. If you were treated at University Health Services (UHS) for a medical or psychological problem, request a "Verification/Evaluation of Medical Care" form from UHS Medical Records office on the 3rd floor of the Tang Center and include this form with your appeal. Please note that it may take up to a week for this form to be completed.
If you were treated by a non-UHS medical professional, you must take documentation from your medical provider to the Medical Records office of UHS to have it evaluated and the above form completed.
If you were seen by a non-UHS psychologist or therapist, please ask your provider to complete the "Summary of Psychological Care by Non-UC Berkeley Providers" form. The sealed and signed envelope containing the form should be submitted with your request.
Significant increases in work hours must be documented on the "Employment Verification for a Reduced Course Load" form. Your supervisor should note how many hours you worked per week before the increase, how many hours you worked after the increase, and the date that the increase occurred. Please be aware that an increase in work hours is not in and of itself sufficient reason for an exception to the deadline.
Faculty letters verifying participation and performance must be written on departmental letterhead and submitted in a sealed departmental envelope. Consult the petition checklist for detailed information.
